Feel better. Begin with a full functional assessment by Dr. Paul. Getting started is easy.
Most of our new patients have sought solutions to relieve their pain and symptoms from other health practitioners or on their own. Others seek to prevent illnesses and promote wellness.
Our primary goal is to determine how our holistic approach can better address your unique concerns and goals. We offer a free 20-minute consultation. Then, depending on the complexity of your symptoms, your initial visit may last between 90 – 120 minutes.
Prior to our initial visit, patients will be required to fill out an extensive history, in addition to brain health and nutrition symptom forms. Links to those forms can be found on this page (give us a call to find out which ones you’ll need). Please fill out below and either print out the forms or email them to us at [email protected].
Dr. Austin is the only practitioner in Boulder County who utilizes the Comprehensive Assessment of Postural Systems (CAPS) Unit to test for postural stability and risk of falling. Most new Functional Wellness patients receive a CAPS test as part of their initial evaluation. Patients stand on a computerized force plate with eyes open and closed, and then repeat testing on a foam pad on top of the plate. This information helps Dr. Austin understand more about core muscle stability, vestibular (inner ear) function, cortical and brain stem function, thus allowing him to approach your treatment more from a brain-based perspective vs. spine-based.
You can complete the forms online and either print the forms at home or email them to us here.

This testing method is used to evaluate the neurocognitive status of patients and covers a range of mental processes from simple motor performance, attention, memory, to executive functions. Immediately following the assessment, scoring is generated into reports, giving Dr. Austin a robust view of the patient’s neurocognitive status. It gives immediate clinical insight into a patient’s current status and level of impairment.

If you are suffering from acute pain or a more chronic condition, our functional wellness approach could be the best way to restore your health and well-being.
The Functional Wellness wheel to the left shows some of the body functions and organ systems that can contribute to dysfunction in the body. So, we look at all of these areas as we work to restore you to health.
We evaluate the negative influences on these systems and work to restore better function and health to the body as a whole.
